The Consortium in partnership with the NCPC (National Council for Palliative Care) is looking to better understand the needs of lesbian, gay, bisexual and trans people around death and dying.
Questions they are looking at include:
Are end of life care services LGBT friendly?
Are there improvements that can be made for LGBT people?
Whether there are any improvements that could be made in end of life care provision.
A spokesman for the NCPC said:
"If you have any experience either personally or through the work that you do, we would very much like to hear what you have to say."
The questionnaire closes on Friday August 13.
